A tournament player verification form is required for EVERY player who is selected to a post season tournament team.

The form must be accompanied by the Original or Certified Copy of player's birth certificate AND

Proof of residency provided by EITHER a properly completed School Enrollment form OR three (3) proofs of residency form 3 different sources as listed on the form.  These proof's must be dated AFTER FEB 1, 2022 but BEFORE FEB 1, 2023. 

The form and back up paper work must then be verified by league player agent/president AND the District Administrator.  

Once a player verification form has been completed and BOTH the league president AND District Administrator have signed it, the form can be used for the remainder of the player's Little League career as long as he does not change residence or schools.  ONE new proof of residency will be required EACH subsequent year--dated between FEB 1 and FEB 1. 

All of these records will be retained by the league EXCEPT the original or certified copy of players birth certificate will be returned after initial examination by the District Administrator.
